You are cordially invited to participate in a product trial conducted by Therapeautic Limited (the “Company”, “we”, “our” and “us”). As your health and wellbeing are our utmost concern, we set out below some of the crucial details about the present product trial, including its background and purpose, procedure, potential risks and discomforts, etc. We very much treasure your voluntary participation. Please review this Consent Form thoroughly before participating in our product trial.

  1. BACKGROUND & PURPOSE OF THE PRODUCT TRIAL 產品試驗的背景和目的

 

The Company intends to launch certain skincare product(s), being a certain kind of face cream with the intended aim to, among other things, soothe or relieve dry or sensitive skin (the “Skincare Product(s)”). In this connection, the present product trial for the Skincare Product(s) (the “Product Trial”) are to:

  1. (a)determine the efficacy and effects of our Skincare Product(s) on a small sample of our target demographic before officially launching them to the market; and

  1. (b)gather and conduct market research on our target market

  1. PROCEDURES 程序

 

The Product Trial will take around a month. The procedural steps will be as follows: -

  1. (a)Before the Product Trial: Before the Product Trial begins, you will be requested to fill out a questionnaire concerning your existing skin conditions. If you have any known allergy, you should immediately inform us in the questionnaire.

 

產品試驗之前：在產品試驗開始之前，您將要填寫一份調查問卷關於您現時的皮膚狀況。 如果您有任何已知的過敏，您應該立即在問卷中告知我們。

 

  1. (b)During the Product Trial: You will apply a thin layer of the Skincare Product(s) on your face at least 1-2 times daily for 3 weeks.

  1. After the Product Trial: You will be requested to fill out a questionnaire for your feedbacks on our Skincare Product(s).

You should always follow the direction and recommendation of our designated staff in carrying out the above procedures. Throughout the Product Trial, we will endeavor to provide customer support to answer any questions from you by WhatsApp at 9197 7859 or by email at info@therapeautic.com.

  1. POTENTIAL RISKS / DISCOMFORTS AND THEIR MINIMIZATION 潛在風險/不適及減少風險的措施

 

Although the Skincare Product(s) are generally safe, there may be some possible side effects, such as:

  1. (a)causing burning, stinging, itching, redness, tingling, and/or dry skin for some people

對某些人造成灼熱、刺痛、瘙癢、發紅、刺痛和/或皮膚乾燥

  1. (b)potentially exacerbating a pre-existing eczema condition for some people

  1. causing allergic or irritant reaction in the skin for some people

  1. (d)causing acne outbreaks for some people

In addition, please note that if you have extremely dry skin or a compromised or damaged skin barrier, you may experience a tingling, stinging or burning sensation after applying the Skincare Product(s). This sensation is temporary and should not last more than a few minutes. Such sensation does not necessarily mean you are experiencing an allergic reaction. However, at no time should you use or apply our Skincare Product(s) over your skin that has been allergic or otherwise compromised or damaged.

To minimize the above potential side effects, we recommend you to first perform patch testing before carrying the above procedures under paragraph 2. For patch testing, you should apply the recommended dosage of the Skincare Product(s) at a test spot where it will not be rubbed or washed away, such as the underside of your arm or the bend of your elbow, and wait if there is any allergic or adverse reaction until the next day. If you feel any allergic or adverse reaction, please immediately cease using the Skincare Product(s) and report to us.

Please note that your participation is on an entirely voluntary basis. This means that subject to the terms and conditions hereof, you may choose to stop at any time.
